NAD Supplement vs Injection: What Each Costs per Month

An over-the-counter NAD supplement is often a precursor sold as a dietary supplement; an injection is a compounded drug on prescription. What prescription NAD+ costs by form, and what each route buys.

What a supplement is

Many NAD products on a store shelf hold a precursor, not NAD+. Nicotinamide riboside (NR) is one; the trial below calls it an NAD+ precursor vitamin[1]. Nicotinamide mononucleotide (NMN) is another.

NMN’s status changed. FDA had said NMN was excluded from the supplement definition[2]. On September 29, 2025, FDA reversed that position[2].

FDA was also clear about limits. Accepting a filing is not a finding that the ingredient is safe[2].

What an injection is

Injectable NAD+ is a compounded drug, on prescription. NAD is in FDA’s 503A Category 1[3]. That list holds substances under evaluation[3].

So both routes skip FDA approval. They skip it in different ways. The supplement is sold under supplement rules. The injection is compounded for one patient.

What the evidence says about raising NAD+

One randomized, placebo-controlled crossover trial tested NR in healthy middle-aged and older adults[1]. It ran two 6-week periods[1].

NR was well tolerated[1]. It elevated NAD+[1]. The authors called effects on blood pressure a question for future trials[1].

That is a measure of NAD+ levels. It is not proof of a health benefit. No trial cited here compares an injection with an oral precursor.

Who pays

IRS rules exclude most supplements from medical expenses[4]. The exception is a supplement a practitioner recommends to treat a diagnosed condition[4].
