What you are pricing
PT-141 is the research name for bremelanotide. The approved product is Vyleesi, a 1.75 mg single-dose autoinjector[1]. It ships in cartons of 2 or 4 pens[1].
That packaging sets the unit. You buy doses. A monthly cost is doses used times the price per dose. The same arithmetic applies to any peptide therapy cost.
The label sets the ceiling. No more than one dose in 24 hours[1]. More than 8 doses a month is not recommended[1]. So the most a label-following month can cost is eight doses.
Who the label covers
Vyleesi is indicated for premenopausal women with acquired, generalized hypoactive sexual desire disorder[1]. It is not indicated for postmenopausal women or for men[1]. It is not indicated to enhance sexual performance[1].
That matters for cost. A prescription outside the label is off-label. A plan is less likely to pay for it.
Why insurance rarely helps

Desire scores rose 0.35 points more than placebo in the integrated analysis[2]. Distress scores fell 0.33 points more[2]. Nausea, flushing and headache hit 10% or more in both trials[2].
The label adds a stop rule. Discontinue after 8 weeks if symptoms have not improved[1]. Budget for eight weeks before judging the spend.
