Formblends is an online prescription shop. It sells sermorelin, NAD+, glutathione, PT-141 and an NAD+ nasal spray. It also sells compounded semaglutide and tirzepatide. Independent clinicians review each intake. Its GHK-Cu comes only as an injection, so it is not priced here. - Prepaid terms are optional. A one-month plan exists on every product. Sermorelin also sells as three months for $477 one-time. Six and twelve months are $954 and $1,908. Longer plans are paid up front and ship every three months.
- Cancellation
- To cancel, contact support by email before the renewal date. There is no account button for it. Cancellation takes effect from the next cycle. One-time plans do not renew at all. Dispensed medication is not refunded. A full refund applies if the clinician does not prescribe.
