Revive Longevity LLC sells cash-pay compounded treatments from Hallandale Beach, Florida. It takes the payment itself. Its terms say it does not prescribe. Telegra MD's provider network prescribes. Strive Pharmacy in Gilbert, Arizona, compounds. Priced by form here: tirzepatide injection, sermorelin injection and NAD+ nasal spray. NAD+, glutathione and PT-141 are sold too, with the route left to the provider. Hormone products fill out the shop.
Each card prints three figures. Tirzepatide's subscription reads "$299 $279 / every 4 weeks". The struck $299 is the one-time month. The 1 Month Supply is "$299 one-time". The card says "Save $20 every month". But it bills every 4 weeks, not monthly. Thirteen charges of $279 come to $3,627 a year. Twelve one-time months at $299 come to $3,588. So the headline and the calendar month differ, and $299 is the figure ranked here. Sermorelin runs the other way. Its $179 subscription is $193.92 a calendar month. That beats the $199 one-time month. NAD+ Nasal Spray is $159 every 4 weeks, or $172.25 a calendar month. Dose does not move the price. The FAQ says: "Your price stays the same regardless of your dosage."

- The 3 Month Supply is the prepaid option. It is one charge up front. Tirzepatide's is $749, marked "BEST VALUE · SAVE $148". That is $249.67 a month, if it lasts three months. Sermorelin's is $449. NAD+ Nasal Spray's is $399. None is required. Every product also sells a single month. A separate membership menu bills every 4 weeks too. Tiers are $299, $599 and $999. Each bundles products and starts with a booked call.
- Cancellation
- Subscriptions stop from your account in the patient portal, or by email. The stop takes effect at the end of the paid period. That period is not refunded. Cancel after provider review but before compounding, and up to $50 may be kept. Once compounding starts, nothing is refunded. A declined applicant is never charged. The card hold is released. Damaged or wrong orders get a reship or credit, never cash. Report them within 48 hours.
- Shipping
- Standard shipping is free on every order. The policy says you will never pay a separate shipping fee. Most orders arrive 5 to 10 business days after approval. Product cards say they ship in 1–2 days. Carriers are USPS, UPS or FedEx. Cold packs go with temperature-sensitive medication. Those cannot ship to a post office box. Delivery is within the United States only.
