Trulicity is dulaglutide, a weekly GLP-1 pen from Lilly[1]. It is labeled for type 2 diabetes[1]. Weight loss is not on the label. Lilly’s weight-loss brand is priced in Mounjaro vs Zepbound.

What a month holds
A carton holds 4 single-dose pens[1]. One pen is one weekly dose[1]. Lilly’s card terms define a month as 28 days and up to 4 pens[3].
A 28-day month means 13 fills a year. At $389, that is $5,057. Twelve fills would leave you short.
The dose does not change the bill
The label starts at 0.75 mg weekly[1]. After 4 weeks it may go to 1.5 mg[1]. Further steps are 1.5 mg, each after at least 4 weeks, up to 4.5 mg[1].
The two top doses were tested in AWARD-11[4]. It enrolled 1,842 adults with type 2 diabetes on metformin[4]. At 36 weeks, HbA1c fell 1.77 points on 4.5 mg and 1.54 on 1.5 mg[4]. Weight fell 4.6 kg and 3.0 kg[4].
Nausea ran 16.4% on 4.5 mg and 13.4% on 1.5 mg[4]. Six of its nine authors worked for Lilly[4]. On LillyDirect, both doses cost the same[2].
With insurance: the savings card
With a commercial plan that covers Trulicity, you may pay as little as $25[3]. That holds for a 1-, 2- or 3-month fill[3].
Savings cap at $150 a month[3]. The yearly cap is $1,950[3]. The card allows 13 fills a year and ends December 31, 2026[3].
At the floor, 13 fills cost $325. A high copay can exceed the cap and cost more.
Free through Lilly Cares
Lilly Cares lists Trulicity[5]. Its site says new Trulicity applications are accepted again, with no medical exception request[5].
You must be uninsured or on Medicare Part D[5]. Household income must be at or below 300% of the poverty line[5]. For one person that is $47,880[5]. The rest is in Lilly Cares.
Against other GLP-1 pens
