Zepbound self-pay starts at $299 for 28 days. That is the 2.5 mg starting dose. From 7.5 mg up, each dose costs $449 if you refill on time[1].
Zepbound is Lilly's brand of tirzepatide. The figures below come from LillyDirect, Lilly's direct-to-patient pharmacy service. It fills orders through licensed third-party pharmacies[1]. The 45-day rule sets the real price
Lilly calls the lower price the Journey Program. It applies from 7.5 mg upward[1]. Refill more than 45 days after the last delivery and it lapses[1].
The late-refill price runs up to $699 a month[1]. A missed refill is not a small slip. It can add $250 to that month.
Pen, vial or KwikPen
The label lists all three presentations[2]. A single-dose pen holds one dose with a built-in needle. A single-dose vial holds one dose. A KwikPen holds four weekly doses[2].
Vials need a needle and syringe for each dose[1]. The KwikPen needs a new pen needle each week[1]. Neither comes in the carton[1]. Add those supplies to the monthly cost.
Used needles go in a sharps container, another cost to plan for.
The single-dose pen is not on the self-pay list. It is priced through insurance and Lilly’s savings card instead[1].

Which dose to price
Price the maintenance dose, not the first month. The label calls 2.5 mg a starting dose only[2]. For weight reduction, maintenance is 5, 10 or 15 mg weekly[2].
For sleep apnea, maintenance is 10 or 15 mg[2]. Steps rise by 2.5 mg after at least 4 weeks[2]. So most of a year is billed at the maintenance price.

The gap at the top dose is $200 a month. The compounded median is the lower figure. Seller prices were read through September 2026.
The two are not the same product. FDA declared the tirzepatide shortage resolved on December 19, 2024[3]. It gave state pharmacies until February 18, 2025 to stop compounding copies[3].

Medicare’s GLP-1 Bridge began July 2026. It covers the Zepbound KwikPen only[4]. The copay is $50 a month for those who qualify[4].
