With commercial coverage for the pen, you may pay as little as $25 [1]. The card pays up to $100 of a one-month fill[1]. So the $25 price holds only up to a $125 copay.

Who qualifies
You need a prescription for an approved use on the label[1]. You must be 18 or older and live in the United States or Puerto Rico[1].
You cannot be in any government-funded program[1]. The terms list Medicaid, Medicare, Part D, Medicare Advantage and Medigap[1]. They also list DoD, VA, TRICARE and state drug assistance[1].
Why government plans are left out
HHS’s inspector general calls a copay coupon remuneration to induce a purchase[2]. When a federal program pays for the drug, the anti-kickback statute is implicated[2]. So coupons typically bar federal program members[2].
The government’s argument is about cost. A coupon can steer buyers toward pricier brands[2]. It also removes a check on drug prices[2].

The card covers up to 13 fills a year[1]. Above the caps, you pay the difference[1]. A $225 copay, for example, leaves you $125 a month.
When your plan does not cover it
If your plan excludes the pen, you may pay as low as $499 per 28-day fill[1]. That applies for up to 13 fills a year[1].
If your plan excludes the KwikPen, a separate schedule applies[1]. It allows up to 11 fills a year[1].

The label lists vials as a third presentation[3]. The card does not cover them. Its terms say other methods of administration are not eligible[1].
The fine print that costs money
- The card expires on December 31, 2026.
- Lilly may change caps or eligibility at any time, without notice.
- Plans using an alternate funding program are barred from the card.
- You must meet the terms every time you use it.

The card is closed to you. Medicare’s GLP-1 Bridge began July 2026[4]. It covers the Zepbound KwikPen only, at $50 a month[4].
