The first two boxes, then the standard price
New self-pay patients get an opening price. It is $199 for each of the first 2 boxes[1]. It covers the 0.25 mg and 0.5 mg doses only[1].
That matches the label’s schedule. Weeks 1 to 4 use 0.25 mg. Weeks 5 to 8 use 0.5 mg[3]. So the opening price covers the two starting months exactly.
The third box reverts to $349[1]. The offer must be redeemed and filled by the end of 2026[1]. Price month three, not month one.
A box is 28 days, not a month
The maker defines a month as one box of four pens. That is a 28-day supply[1].
Thirteen boxes cover 364 days. So a calendar month at $349 a box works out to $379. Compare that figure with a seller who bills by the calendar month.

The higher dose costs more
The label allows a 7.2 mg weekly dose for weight reduction in adults[3]. It applies after at least four weeks at 2.4 mg[3]. That pen is Wegovy HD, at $399 a box[1].
With insurance, and with Medicare

What a telehealth seller adds
Branded semaglutide bought through a telehealth seller has a typical monthly price of $286, across 12 sellers. Some of those figures add a program fee to the drug, and some quote an opening price. Check both before setting one beside the maker’s $349.

Compounded semaglutide is a different purchase
Compounded drugs are not FDA-approved[4]. FDA does not review them for safety, effectiveness or quality before sale[4].
FDA recommends using a multi-dose vial within 28 days of first use[4]. A cheap vial that outlasts that window wastes drug. Whether any generic exists is covered in generic semaglutide.
