What the program is
The offer started on February 23, 2026[1]. It applies automatically on your first purchase of a qualifying dose[1]. New and existing patients are both eligible[1].
One supply is 28 days[1]. For the vial, that is four single-dose vials[1]. For the KwikPen, it is one pen[1]. Lilly calls this a “1-month supply”. It is four weeks, not a calendar month. How that unit changes a yearly total is in billed every 4 weeks vs monthly.

The 45-day window, counted the way the terms count it
The terms set two dates. The start is “the delivery/received date” of your previous vial or KwikPen order[1]. The end is when you “complete your refill purchase”[1]. That gap must be 45 days or less[1].
A supply lasts 28 days. So the window runs 17 days past the end of one supply. Those 17 days are your whole margin. A missed reminder, a card decline or an empty prescription eats them.

What eats the slack
Lilly’s own order guide gives the steps and their timing[4]. Confirmation takes up to 1 business day. A status update follows within 2 business days. A checkout link follows within 2 more[4].
That is up to 5 business days before you can pay, on a new prescription. Delivery averages 3 business days after checkout[4]. The guide says the pharmacy reaches out about the refill within 23 days[4].
The purchase date is what counts, not the delivery date[1]. On that wording, a refill paid on day 44 qualifies, even if it arrives later. A refill that needs a new prescription is the slow case. Your prescriber must send one first[3].
If the refill runs late
A late refill is billed at the “Regular price”[1]. At 7.5 mg the lapse adds $50. At 10, 12.5 and 15 mg it adds $250. Those figures come from the table above.
The terms do not say whether the offer returns on the next on-time refill. They say only that the offer “will not apply” to the late one[1]. Ask LillyDirect before you assume it resets. Its refill line is printed on the Zepbound page[2].
The KwikPen self-pay card has a safety net the vial does not. Say you refilled on time and the pharmacy could not verify it. You can file a claim for the difference[1]. Claims must be in or postmarked by March 31, 2027[1]. You need the original pharmacy receipt and your card details[1].

The fill cap matters over a full year. 28-day supplies fit 13 times into a calendar year. The KwikPen card covers 11[1]. The terms do not say what the rest cost. The card itself “expires and savings end” on December 31, 2026[1].
Costs the program does not cover
Needles are extra for both the vial and the KwikPen[2]. They can be added at checkout for a fee[3]. The terms add that “additional taxes and fees may apply”[1]. Home delivery is free[3].
