The bill, in one line
$199 a month buys NAD+ at the 500 mg dose on a 90-day supply. Direct Meds lists no membership fee and ships it free.
Direct Meds is a cash-pay telehealth. It sells NAD+ as an injection or nasal spray, sermorelin, glutathione and compounded GLP-1s. A licensed provider reviews a free online assessment first. A compounding pharmacy fills the order. It also lists a research-only peptide this site does not cover, because no pharmacy may lawfully compound it.
The NAD+ figure moves with the dose. At 500 mg, the 90-day supply is $199 a month and the 30-day supply $249. At 1,000 mg, those become $249 and $299. A third 1,000 mg option bills $699 every three months. Sermorelin is $249 a month on a 90-day supply, or $299 for 30 days plus $9.99 shipping. The NAD+ nasal spray is $119 a month on a 90-day supply.
Every price, in the seller’s own unit
A per-vial price stays per vial. A monthly figure appears only where the seller’s unit carries one, and a cost per milligram only where the seller states how much the unit holds.

The fine print
- Prepay
- The 90-day supply ships at once and bills monthly. The product pages do not say what is owed if you cancel before the third charge. Ask before you pick it. The 30-day supply costs more, but you only hold stock you have paid for.
- Cancellation
- Turn off auto-refill in your account at least 48 hours before the next processing date. Subscriptions renew every 30 days, with an email notice 5 to 7 days before each charge. There is no refund for medication already received. A return, where one is accepted, carries a $35 disposal fee.
- Shipping
- Shipping is free on the NAD+ plans. Approved orders ship within 1 to 2 business days from a US pharmacy. Direct Meds does not serve Mississippi or Louisiana.
Worth it, and what to watch
Worth it
- No membership fee on any plan
- Free shipping on NAD+ and GLP-1 plans
- Names its compounding pharmacy on the NAD+ page
- Monthly billing even on a 90-day supply
Watch for
- NAD+ price rises with the dose
- Early-cancel terms on 90-day supplies not stated
- No refund once medication arrives
- Not available in Mississippi or Louisiana
- Buy here if
- A buyer starting NAD+ at 500 mg who wants a flat monthly bill with shipping inside it.
- Look elsewhere if
- A buyer who may stop inside three months. The 90-day supply ships up front, and the refund terms are narrow.
