The program fee is published and plain, but the number that matters most, the drug, is set by your insurance plan or your pharmacy, so no total can be worked out before you enroll.
Form Health is a Boston-based virtual obesity medicine practice that bills most major private insurers and Medicare, the way an in-person specialist would. Its clinicians prescribe FDA-approved drugs only, including Wegovy, Zepbound, Saxenda and the Foundayo tablet, so nothing on the site is compounded. The company itself sells no drug; prescriptions are filled by a pharmacy and paid through your coverage or out of pocket.
There is no drug price to compare, because Form Health does not sell the drug. With in-network coverage, you owe the usual copay or deductible on visits, labs and medication, and the FAQ says the team handles prior authorizations. Without coverage, the self-pay plan is $299 a month, and that buys the care only: clinician and dietitian visits, unlimited messaging and the app. Labs and medication are paid separately on top, so the headline and the real monthly cost differ by the full price of the drug. The FAQ adds that Zepbound vials are cash-pay through LillyDirect and never run through insurance.

The fine print
- Prepay
- No prepaid bundle is offered, and the self-pay plan bills monthly. The payment consent stores a card and charges it automatically for whatever your insurer does not cover after each visit, which means the bill can arrive as a card charge rather than an invoice.
- Cancellation
- The FAQ says you can cancel the membership at any time, and the terms say the account closes at the end of the current billing period rather than at once. To revoke the stored-card authorization, contact the billing email at least 15 days before the next scheduled charge. A visit you miss without two business days' notice can cost up to $100.
- Shipping
- Not applicable in the usual sense: Form Health ships no medication. A prescription is filled by your pharmacy, so delivery and any delivery charge depend on which one you use.
