Both start at $349 a box[3][4]. Both are semaglutide from Novo Nordisk. The gap opens at the top dose.
Ozempic’s 2 mg pen is $499[3]. Wegovy’s 2.4 mg box is $349[4]. That is $150 a month more for less drug.
What one box holds
An Ozempic box is one pen[3]. At 1 mg and 2 mg, one pen holds four weekly doses[1].
A Wegovy box is 4 single-dose pens[4]. That is 28 days[4].
| Pen and dose | Per box | What the box holds | Per mg |
|---|---|---|---|
| Ozempic 1 mg | $349 | 1 pen, 4 weekly doses | $87.25 |
| Ozempic 2 mg | $499 | 1 pen, 4 weekly doses | $62.38 |
| Wegovy 2.4 mg | $349 | 4 pens, 28 days | $36.35 |
| Wegovy HD 7.2 mg | $399 | 4 pens, 28 days | $13.85 |
The per-mg column is arithmetic on the box, not a dose. Price the dose your prescriber writes. The cost-per-mg tool runs the same sum for any vial or pen.
Starting prices
New self-pay patients pay $199 for their first 2 Ozempic fills[3]. Wegovy's first two fills cost the same[4].
Both offers cover the two starting doses only[3][4]. Wegovy’s must be redeemed by December 31, 2026[4]. After that, the standard price applies.
The higher doses
Ozempic tops out at 2 mg a week[1]. Wegovy’s usual maintenance dose is 2.4 mg[2]. Wegovy HD adds a 7.2 mg pen at $399[4].
The 2 mg Ozempic dose was tested against 1 mg in SUSTAIN FORTE[5]. The trial ran 40 weeks in 961 adults with type 2 diabetes on metformin[5]. HbA1c fell 2.2 points on 2 mg and 1.9 on 1 mg[5]. Weight fell 6.9 kg and 6.0 kg[5].
Wegovy 2.4 mg was tested in STEP 1[6]. It enrolled 1,961 adults with obesity or overweight and no diabetes[6]. Weight fell 14.9% at 68 weeks, against 2.4% on placebo[6].
Different people, different endpoints. Neither trial compares the two brands.
The pills

The strengths both lists sell cost the same on each[3][4]. The Wegovy pill adds a 25 mg bottle[4]. The ladder is in Wegovy pill cost.
With insurance
Ozempic is labeled for type 2 diabetes[1]. Wegovy is labeled for weight, heart risk and a liver disease[2]. A plan pays against the label.
Ozempic’s card: as little as $25 a month, up to $100 off[3]. Wegovy’s card: as little as $25, up to $100 off a month[4]. Both need commercial coverage of that brand.

Against compounded semaglutide
Compounded semaglutide is priced by each seller. It is not FDA-approved. Each seller’s month is on the semaglutide price board. The all-in board ranks them with fees counted. The full brand ladder is in Wegovy cost and Ozempic cost.
