Fixing Ozempic face means putting back volume or lifting loose skin. Filler is the cheapest way in. A facelift is the most expensive. The gap between them is more than $10,000 in surgeon fees alone.
The term covers the hollow cheeks some people notice after losing weight on semaglutide or tirzepatide. It is not a diagnosis. The figures below are national averages from the American Society of Plastic Surgeons, each with its year. What the pen itself costs is in Ozempic cost.
What surgeons say people ask for
More than four in five ASPS member surgeons reported GLP-1-related consultation requests in 2025 [1]. The report says 82 percent [1].
For the face, surgeons most often reported GLP-1 consultations about facelifts, at 71 percent. Neck lifts followed at 64 percent [1].
Asked which facial rejuvenation approaches they used in 2025, 69 percent named hyaluronic acid fillers. Autologous fat grafting came second at 49 percent [1].
Facial fat grafting rose 39 percent, to 34,117 procedures in 2025. ASPS calls it the largest percentage increase of any cosmetic surgical procedure [1].
Filler: the lowest fee, and the one you repeat
The ASPS 2023 fee table puts hyaluronic acid filler at $715 [2]. Non-hyaluronic filler, such as Radiesse or Sculptra, averaged $901 [2].
Both fell from 2022. Hyaluronic acid filler averaged $794 that year, and non-hyaluronic filler $1,039 [2].
ASPS does not say whether the figure covers one syringe or one visit. Ask the office which one its quote means.
Filler does not last forever. The FDA says some fillers are naturally absorbed, so patients may need to repeat the procedure [4]. How long it lasts depends on the material and the area injected [4].
That makes filler a recurring cost, not a one-time one. Count the visits, not just the first bill.
Fat grafting and lifts: one bill, but a larger one
Facial fat grafting averaged $3,617 in 2023 [2]. ASPS's 2024 table gives a projected range of $3,000 to $5,500 [3].
A facelift averaged $11,395 in 2023 [2]. The 2024 range is $12,000 to $19,000 [3]. A neck lift averaged $7,885, with a 2024 range of $7,500 to $13,000 [2][3].
ASPS switched to ranges for 2024. It says the change reflects differing locations and practice settings among its members [3].
One comparison, by our arithmetic: $11,395 divided by $715 is about 16. A 2023 facelift fee equals roughly 16 average filler fees.

What the average leaves out
The fee is the surgeon's share only. ASPS says its facelift average does not include anesthesia, operating room facilities or other related expenses [5].
Its facelift cost page lists what else can be billed [5]:
- hospital or surgical facility costs
- anesthesia fees
- prescriptions for medication
- post-surgery garments
- medical tests
ASPS adds that a surgeon's fee depends on experience, the procedure used and the office's location [5]. Ask for the full quote in writing, with each line named.
Will insurance pay for any of it?
Almost never. ASPS says health insurance does not cover dermal fillers [6]. It says most plans do not cover facelift surgery or its complications [5].
Insurer policies draw the same line. Aetna treats FDA-approved filler as medically necessary for facial lipodystrophy from HIV antiretroviral therapy. It calls filler cosmetic for all other uses [7].
Medicare contractor policy reads the same way. One local coverage determination covers dermal filler only for HIV-related facial lipodystrophy, under set conditions [8].
Weight-loss hollowing is not on either list. Plan to pay cash, and check whether the surgeon offers financing. Coverage for the drug is a separate question, covered in does insurance cover Ozempic.
Pricing the fix against the medicine
Face work sits on top of what the weight loss already costs. A month of compounded tirzepatide or a brand pen is its own line. See GLP-1 cost without insurance for the monthly figure. The one question to ask at a consultation: what is the total with facility and anesthesia? Then ask how many sessions the quote assumes.
